Tell us who you are, what your household looks like, and roughly what you'd like the first visit to be about. Lena will read it within a business day and send you scheduling options. If you'd rather call, the practice line is (218) 727-0413; she will take down the same information by voice.
02 · Enrollment
Once we've matched a slot to your schedule, we send a one-page intake (your medical history, current medications, allergies, recent labs if you have them) and an enrollment-fee invoice. The fee is $50, waived for members coming from another DPC practice and for sliding-scale members. Payment is by ACH or card; we do not take cash for the enrollment, but we do for ancillary services if you'd prefer.
03 · First visit
The first visit is 90 minutes. It is the longest visit you will have in this practice — subsequent visits run 30–60. We take a careful history, examine you, run any in-office labs we can run that day, write the plan, and book whatever follow-ups make sense. You become a member at the moment of the first visit, not at the moment the form is submitted.

If a federally-qualified community health center is what you need, the Lake Superior Community Health Center serves the Twin Ports on a sliding fee scale and accepts Medicaid. If you need a hospital-based primary-care home, Essentia and Aspirus both run primary-care practices in town.
