A note on the form

Each case log opens with a one-paragraph encounter summary, then a splits table (each clinically meaningful event timestamped against the start of the encounter), then a debrief — what was tracking, what wasn't, and what the next-visit plan is. Where a peer-reviewed source informs the clinical reasoning, the link is in the body and in the references at the bottom. We do not embellish outcomes. Where a case did not go well — and the splits board has those cases too, in time — we say so.

Members who appear in a case log have signed a specific written consent, distinct from the consent to be in the panel. The case log is reviewed by the member before publication. The member retains the right to ask for a case log to be removed, with a thirty-day takedown SLA. We have not, to date, been asked.

The four cases above were chosen, from a longer working set, because each touches a different lane and each surfaces a different clinical principle: the math of GLP-1 deferral, tendinopathy as a loading problem, tracking before treating, and induction at home.
